Transparency Statement

I am a full-time employee of Virta Health, a company dedicated to using an approach of
carbohydrate restriction combined with a mobile app to combat type 2 diabetes. Virta has also
offered me stock options. In addition, I am a compensated advisor for the Simply Good Foods
Company, Inc.
